Financial Policy Addendum

Hope and Inspiration Psychological Services, PLLC

To ensure we can continue providing high-quality care and maintain efficient practice operations, we require all clients to review and adhere to our updated financial policies outlined below.

1. Intake Appointment Deposit Policy

To secure an intake appointment, a $200 non-refundable deposit is required at the time of booking for all self-pay and commercial insurance clients (including Aetna and NC BCBS).

  • $75 goes toward a materials fee.
  • $125 is credited directly toward your evaluation balance.

* Please note: Appointments are not confirmed until this deposit is received in full.

2. Payment Responsibility

All outstanding balances—including copays, deductibles, and coinsurance—are due in full at the time of service.

3. Late Payment & Interest Policy

Accounts not paid in full by the due date (or within 30 days of the statement date) are considered past due.

  • Past 30 Days: Balances past 30 days will be assessed a late fee of $25.00 or 1.5% of the unpaid balance per month.
  • Past 60 Days: Unpaid accounts past 60 days may be forwarded to a third-party collection agency or pursued through legal action. Clients assume responsibility for all associated collection costs.

4. Card on File Authorization

By signing our financial agreement/addendum, clients authorize Hope and Inspiration Psychological Services to charge the credit/debit card on file for any outstanding copays, deductibles, or unpaid balances that are 30+ days past due.
