Forms and Fees
The Child Intake Form/Social History and additional forms will be sent through Therapy Notes. Please look for an email from jepratt@
hopeandinspirationspsych.com
for the forms for your intake appointment.
- Currently, Hope and Inspiration Psychological Services takes Medicaid for: Partners, Alliance, Vaya. Amerihealth, Carolina Complete, and Wellcare
- Commercial Insurance accepted includes Aetna and BCBS NC
- Those with commercial insurance are obligated to pay their copay per their insurance agreement. Please check with your insurance company prior to your scheduled appointment.
For Self-Pay Evaluations ONLY
This excludes those using Medicaid
- Referral/Intake Appointment In person, phone call, or virtual $150.
- At the referral appointment, assessment needs will be determined, and a Good Faith Estimate given. A testing appointment will be set.
- Testing and report writing will be $150/hour. The feedback session is $150, at which time the report will be emailed after the session.
- If additional testing is required, the scope and price will be determined and communicated.
- Beginning July 1st, 2024, there will be an added $75 materials fee for those who self-pay or are out-of-network to help cover the cost of testing materials.
